Changes to registration during Covid-19 Pandemic
Due to the current COVID-19 pandemic, patients are asked to register without coming into the practice. To register you will need to complete our online regsitration form
How To Register
To register at The Vale Medical Centre, you need to complete the GMS1 Registration form along with the Patient Questionnaire.
Please hand in the completed forms to the reception team who will process your application within 4 working days. You can put your completed registration forms in our post box which is attached to our main gate. Please either staple or put into an envelope so that all pages are kept together.
Registering with the Practice
Please come to the surgery to register during the following times:
- Mondays between 9:30am to 6:00pm
- Tuesday between 9:30am to 7:00pm
- Wednesday between 9:30am to 7:00pm
- Thursday between 9:30am to 7:00pm
- Friday between 9:30am to 6:00pm

Named Accountable GP for all Patients
You may be aware that all GP Practices are now required to provide their patients with a 'named, accountable GP'
As stated on the BMA website:
The contract requires the named accountable GP to take responsibility for the co-ordination of all appropriate servies required under the contract and ensure that they are delivered to each of their patients where required (based on the clinical judgement of the named accountable GP).
All of our patients here at The Vale Medical Centre, including children, now have a named GP who is responsible for their overall care. Patients will start to be notified when they contact the Surgery, on repeat medication slips and by text or e-mail where available.
Please note that this does NOT prevent you from seeing any GP of your choice in the practice as you currently do now, and you are unlikely to see any notable change in the way care is delivered to you by us.
